If I want to close my business what is the procedure?
You may close your business through the use of several forms. (1)The renewal form – mailed in September of each year and due by October 31st of each year. (2) The license bill – mailed in December of each year and due by January 31st. Each form has a place that allows you to indicate if you wish to renew your business.

You may also down load the Close Account form from our website and send it in either by regular mail, e-mail, or fax.

To cover both you and our staff member, all forms must be signed by the owner listed on the application in order to close your business.
